Obama addresses health care with AMA

CHICAGO -- President Obama took his case for overhauling the health care system and allowing people to buy into a government-sponsored insurance plan directly to doctors Monday, delivering a nearly hour-long speech to the American Medical Association.

Blaming the fear tactics of the past for the current failures of the health-care system, Obama repeatedly told his audience that no one would be forced to give up their private insurance.

"I know there are millions of Americans who are content with their health care coverage," he said. "No one will take it away. No matter what."

Obama has made restructuring the health-care system a top priority of his administration, calling it "the single most important thing we can do for America's long-term fiscal health" and urging Congress to get a bill on his desk by October.

The audience at the annual meeting of the powerful physicians' group -- whose leader last week expressed concerns about a public plan -- interrupted the president frequently with applause. They clapped when he said overhauling the system would allow doctors to be healers instead of "bean-counters and paper-pushers," that more should be done to reward medical students who choose to become primary-care physicians or work in underserved communities and that research on best practices was not meant to dictate the kind of care that should be provided.

The most notable moment came when Obama acknowledged that change would be difficult if doctors feel like they are "constantly looking over their shoulders for fear of lawsuits."

The crowd applauded, but then Obama warned, "Don't get too excited yet."

Those cheers turned to boos when he went on to say that he was not advocating a cap on malpractice awards, which many doctors support, because such caps could be unfair to people who have been hurt.

"I want to work with the AMA,” he said, “so we can scale back the excessive defensive medicine that reinforces our current system and shift to a system where we are providing better care simply -- rather than simply more treatment.”

Still, the speech was billed as a "big-picture" look at the kinds of changes Obama wants to see in the health-care system and -- just as he did last week at his first town hall on the issue in Green Bay, Wisc. -- the president offered mainly broad guidelines, rather than specifics.

He said making health care affordable for everyone would cost some $1 trillion over the next decade and then went on to spell out some $950 billion in estimated cost savings that could come from introducing competitive bidding into the Medicare Advantage Program, changing how Medicare reimburses hospitals, encouraging generic drugs and other measures.

He said these savings would offset the cost of health care reform over the next 10 years, an argument he made in his weekly radio address on Saturday.

Obama again used the recent bankruptcies of two of America's Big Three carmakers to link the issue to the economy, though he went a step further today than he did in Wisconsin to drive his point home.

"A big part of what led General Motors and Chrysler into trouble in recent decades were their huge costs they racked up providing health care for their workers; costs that made them less profitable and less competitive with automakers around the world," he said. "If we do not fix our health care system, America may go the way of GM; paying more, getting less and going broke."

House Republican Leader John Boehner (R-OH) put out a statement after the speech saying he was pleased that Obama had addressed the issue of medical malpractice.

"We're pleased the President has expressed rhetorical support for medical liability reform,” Boehner said, in part, “but fact is the effect of even the strongest medical liability reforms would be negated by a government takeover of health care that raises taxes, rations care, and drives health care costs even higher. If President Obama is serious about reducing the skyrocketing cost of health care, he'll press his own party to include real medical liability reform as part of a reform package that puts patients and doctors in charge of their health care rather than putting government in charge."

Boehner's statement came after a conference call before the speech, hosted by the Republican National Committee, in which Georgia Congressman Tom Price (R), who is himself an orthopedic surgeon, equated a government-backed health-insurance option with a government "takeover" that would put private insurers out of business.

Another great speech by President Obama.  We need to get the AMA out of our wallets by having public option health care insurance.  The reason that the private insurance ripoff conartists are against a public option is that they don't want competition when it comes to ripping off real Americans.  The AMA cares about capping medical legal liability but not on their outrageous const increase over the past few decades.  The AMA is the main culprit so many Americans file for bankruptcy even when they are insured.

Listen Obama, you fooled the people of the country once with you stimulus bill. You said "If we don't pass it now, are economy will see over 9% unemploymnet". So the bill was passed, and we see 9%+ unemployment.

Hmmm. Who again is using fear to get their way?
What a liar Obama is! The Democrats are OWNED by the Trade Unions and the Trial Lawyers.

It is nothing but empty rhetoric about medical tort reform, liability caps, malpractice insurance reform, product liability limitations, and why? The Trial Lawyers Association is the second largest PAC contributor to the Democratic Party. And remember Obamaphiles at MSNBC, the Trial Lawyers will eventually name GE Healthcare and GE Medical Systems as party to some multimillion dollar frivolous lawsuit. There goes Immelt's bonus!

Millionaire Democrats (the seven wealthiest US Senators for example) find it easy to call for National Heath Care because 1) They won't be paying for it, all their money is in tax sheltered trusts; 2) Nor will they be using it. Uncle Sam pays for what they don’t pay for out of their vast fortunes.

The next time Teddy or Kerry or Kohl or Lauetnberg or Rockefeller or Obama or Clinton or Gore or Daley need an operation, they will be airlifted to a private room at Mass General or the Mayo Clinic where no expense will be spared. Crumbs for you and your family as you sit next to an illegal alien in a public hospital.

'...TRADING LAWSUITS FOR A PUBLIC OPTION?....

There was a paragraph in President Obama's speech the AMA this afternoon that the audience is likely to ponder quite a bit:

"I recognize that it will be hard to make some of these changes if doctors feel like they are constantly looking over their shoulder for fear of lawsuits. Some doctors may feel the need to order more tests and treatments to avoid being legally vulnerable. That's a real issue. And while I'm not advocating caps on malpractice awards -- which I believe can be unfair to people who've been wrongfully harmed -- I do think we need to explore a range of ideas about how to put patient safety first, let doctors focus on practicing medicine, and encourage broader use of evidence-based guidelines. That's how we can scale back the excessive defensive medicine reinforcing our current system of more treatment rather than better care."


For the American Medical Association, in particular, presidential acknowledgement of "excessive defensive medicine" will no doubt be welcome.

The comments come on the heels of an NYT report on the White House's willingness to consider changes to the system surrounding medical malpractice lawsuits.

In closed-door talks, Mr. Obama has been making the case that reducing malpractice lawsuits -- a goal of many doctors and Republicans -- can help drive down health care costs, and should be considered as part of any health care overhaul, according to lawmakers of both parties, as well as A.M.A. officials.

It is a position that could hurt Mr. Obama with the left wing of his party and with trial lawyers who are major donors to Democratic campaigns. But one Democrat close to the president said Mr. Obama, who wants health legislation to have broad support, views addressing medical liability issues as a "credibility builder" -- in effect, a bargaining chip that might keep doctors and, more important, Republicans, at the negotiating table.



OK, but what might the president get in return? If lawsuit limits are a bargaining chip, what can Obama expect the AMA and Republicans to bargain on?

Here's an idea: the White House opens the door to reducing malpractice lawsuits and detractors make room for a public option. For the AMA, opposition to a public plan doesn't really make any sense, and seems to be driven by a bizarre concern for the insurance companies.

If the AMA can get movement on medical liability issues out of the administration, maybe the public option might not look so outrageous after all?

Post Script: Keep in mind, as long-time Monthly readers may recall, that the very idea of a "lawsuit crisis" is patently ridiculous, but that doesn't change the fact that it remains a high priority for opponents of reform...'

Ahh, the lawyer lobby got to Obama before the big AMA speech! The little floater Obama put out there about capping lawyers fees just didn't sit well with the lawyers lobbying groups. I'm sure they were on the phone talking to Rahm Emanual reminding Rahm to just where all that campaign money Obama got last year  came from. The teleprompter then got the call from Rahm to change the text.

And it's not too hard to figure out why. The Plan will cost so much, common people will be crushed about how much free health care is going to cost them.

There is a lot of room for compromise and the AMA will ultimately be satisfied.  Actually, I am confident Congress will pass healthcare legislation this year...And a large portion of it will be in place by 2012. The Obama administration is working to help main street...and main street will reelect Obama in 2012. Likewise, the economy will improve...enough.

This is a stupid ass...this one doesn't realized that HCR will include the reformation of malpractice insurance as well. This is a "primary" culprit of physicians profits. Since it is so, the physicians have to recoup thoses expenditures from some place. Just where does he thinks those profits come from.

Yet he comes right back with his party of "NO" rhetorical speech. He is a joke. LOL
For the longest time, i was under the illusion that my health is great and that i dont give a hoot about anyone who has or does not have healthcare coverage. Well, it has hit my family and reality is begining to set in, because i am already in fight with the insurance company who tells me that i need to pre certify before i get sick and i need to call at 8:49 pm so that they can approve the ambulance cost first. The Charge so far for preliminary tests are already clocking $12,005.67. We still have surgery and rehabilitation. Me and my wife are on my company's group plan and as it stands right now, the plan wants to see the bill and they will decide what they pay and what we pay. Did i mention that i have been paying premiums for 15 years without any serious health issues. Look, my piont is that unless it hits you, this whole healthcare crap sounds fun to easily dismiss. I hope disease is as prejudice as we all sound in the comments.
No plan was presented by Obama. Just more pie in the sky meaningless rhetoric, which Obama is good at giving.
So lets see if we have this straight. We're going to spend trillions so we can save billions?

Can someone have Obama run a business, even if it's for just a day or two? Doesn't someone have a Take Your President to Work Day or something? That way Barack can get a real world view that a business needs to make money, not lose it, to stay in business.
One item I have not seen mentioned by any of the posters so far.  Idea!!! IF there are thousands of citizens not covered by insurance/prescription drug plans now, when they ARE covered (by any means) this will mean thousands of NEW Customers for doctors, hospitals, and drug companies.  Looks like a win-win.
Regarding those who insist that the effectiveness research that Obama wants to do to help lower costs for unnecessary tests and treatments is a prelude to the government rationing health care.  I would rather have the government deny a procedure because it doesn't work, than an insurance executive denying it because they don't want to pay for it even though it does work.
Naturally the orthopedic surgeon in Ga is going to be against anything like this.  Duh.

Hey how about giving primary care docs a higher rate of reimbursement?

A dermatologist can spend 5 minutes with me and charge $300 bucks but my internist who takes care of my heart disease, arthritis, and depression, spends 40 minutes with me and writes 6 prescriptions only charges $105.

Increase remuneration to primary care and decrease it to subspecialists like Orthopedic surgeons.

Everyone who is against this change obviously has healthcare access.

What about the folks that don't?  What are they saying?

If an insurance company denies a procedure, you can always go to another insurance company. If the government denies a procedure, where do you go then?
I had the BLESSING of a National Health Care System most of my life and now am in the USA. Health care here is really scary. I have noticed that in all those countries with a National system, people are not dying like flies. Neither are they bankrupted or denied health care if they lose their jobs. Once here, in the US, those who are loudest in their opposition, will NEVER EVER give a National system up.
Obama couldn't operate an ICE-CREAM stand yet alone a large corporation.  So why do we trust the doofus to run the country???

BIG government exchange money with AIG ONCE without the public getting screwed... and now they want to engage the government in millions of transactions a year???

It's like trying to steal a bottle from a giant baby's mouth once they start sucking.

But what does your baby-like dependence on Socialistic health care have to do with Freedom and Liberty?  Absolutely nothing...  You like to be diapered now and forever, once coddled, never free.
To all you nay-sayers, try using your brains rather then that lower part of your anatomy to think with. Right now trillions of dollars are being spent collectively for Medicare, Medicaid, and private health insurance. Those who do not have insurance, and have health crises, go to the ER, receive needed care, and the hospitals, doctors, and insurance companies ALL collectively pass along those unreimbursed costs to those of us who DO pay large health insurance premiums, and the taxpayers in general. Providing health insurance to everybody is NOT going to cost the average Joe-Blow anything more. It simply means that those who do not have health insurance now, will have to pony up something for their what is free now health care. In other words, spread it around a little bit. Personally for me, after I pay my $900 a month health insurance premiums for me and my family, that sounds like a GOOD thing.

What works in the postage stamp countries of Europe won't work in the USA. And don't tell me about Canada, the population is much smaller than the US and the taxes are a lot higher.

If you don't like the US health system, you are free to go back to whatever socialist paradise you left.
